Description Easterseals Blake Foundation started as the Cerebral Palsy Foundation of Southern Arizona. Since then, we have expanded our services to better serve our community members.

Key Responsibilities

• and Responsibilities: Assesses the member’s behavioral health needs and eligibility for services.

• Assists staff in developing and monitoring member service plans.

• Provides comprehensive intakes, assessments, annual assessments, and offers therapeutic and support services.

• Screens, evaluates, and schedules referred members who may need behavioral health services.

Requirements

• TR to diagnose older children, adolescents and adults and the crosswalk to the DC

Benefits & Compensation

• Hourly rate: $24/hr.

Work Schedule

• Full-Time position.

• Weekend coverage may be required.
