Key Responsibilities
- Participates in treatment planning, care coordination, discharge planning, and clinical follow
- Works holistically to meet the physical, social, and emotional needs of clients.
- Sensitivity to the needs of parents/caregivers and the ability to support and involve them in treatment plans.
- Maintains accurate and timely clinical documentation in electronic medical record systems.
Requirements
- Persons of First Nations ancestry will be given preference (OHRC Part II Special Employment).
- Ability to communicate in one of the First Nations dialects in the Sioux Lookout zone is an asset.
- Bachelor’s degree in social work (or a related field); master’s degree considered an asset.
- Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.
