Key Responsibilities

Participates in treatment planning, care coordination, discharge planning, and clinical follow

Works holistically to meet the physical, social, and emotional needs of clients.

Sensitivity to the needs of parents/caregivers and the ability to support and involve them in treatment plans.

Maintains accurate and timely clinical documentation in electronic medical record systems.

Requirements

Persons of First Nations ancestry will be given preference (OHRC Part II Special Employment).

Ability to communicate in one of the First Nations dialects in the Sioux Lookout zone is an asset.

Bachelor’s degree in social work (or a related field); master’s degree considered an asset.

Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.
