Key Responsibilities

• provide the highest level of continuity of care for the patient.

• implement the patient’s Plan of Care in use of therapeutic and self care activities for the purpose of improvi.

• provide support at every stage of life's journey.

Requirements

• service training programs, on

• Up to 8 hours/day Standing

• Up to 5 hours/day Walking

• Up to 2 hours/day Driving

Work Schedule

• Per Diem position.

• Weekend coverage may be required.

• Night or evening shifts may be required.
