Position Summary

Responsibilities
Social Worker-Palliative Care
Will be providing social work services in our Outpatient Palliative Care.
*Responsible for provision of social work services to include explanation of palliative care services, crisis intervention, resource referral and emotional support and/or counseling to patients and families as needed.
*Lead support groups.
*Ability to work in coordination with other disciplines at part of team to address chronic illness and end-of-life needs.
Qualifications
Palliative Care Social Worker Qualifications
*Masters Degree in Social Work required. NC LCSW, OR LCSW Provisional license preferred. *Must obtain provisional license after hire. *At least 1 year experience in social work and knowledge of end-of-life issues/medical social work preferred. *Reliable transportation and valid NC Drivers license required. *New Grads Welcome to Apply! *Some travel locally will be required, generally within a 10-mile radius. *This position does count toward clinical hours for a LCSW license.
