The VNA & Hospice of the Southwest Region is looking for a per diem Speech Language Pathologist (SLP) for Rutland County. Key Responsibilities
- Performs a comprehensive speech and language physical, psychosocial, and safety assessment as evidenced by documentation in the clinical record.
- Perform initial and ongoing assessments of patient's communication and swallowing status.
- Develop and implement individualized treatment plans based on physician orders and patient needs.
- Provide direct speech, language, voice, fluency, and swallowing therapy services in the patient's home.
Requirements
- Master's Degree in Speech
- Language Pathology from an accredited program.
- Current licensure as a Speech
- Language Pathologist in the state of practice.
