FQHC/340B experience is beneficial Licensure / Certification: Current state of Idaho Pharmacy license and drug screen completed prior to start date.

Credentialing Meets established Credentials Committee requirements for PharmD scope of practice including fully credentialed, licensed and privileged as well as the ability to pass a background check.
