This position qualifies for federal student loan forgiveness. After 10 years of qualifying payments while working full-time for a nonprofit employer, the remaining balance on your Direct Loans may be forgiven.
About the Role
Join Housecall Providers as a Hospice Social Worker - PRN in United States. Hospice Social Worker - PRN --------------------------------------------------------------- NOTE: This is a PRN role that will work 100% in the community and support the Portland Metro area. Candidates will need to have 5 weekdays of availability per month.
Key Responsibilities
- Performs initial and ongoing psychosocial/bereavement evaluations of patients/family/caregiver and assists in the development/implementation of the plan of care.
- Is a core member of the interdisciplinary group (IDG) and actively participates in IDG meetings for development and evaluation of the patient’s plan of care.
- Implements a plan of care for each assigned patient/family/caregiver unit and evaluation of that plan of care.
- Assists patient/family/caregiver in determining of goals of care including completion of Advance Directives and POLST forms as needed.
Requirements
- Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle
- CPR certification at hire or within 6 months in position Knowledge, Skills and Abilities Required Knowledge
- Knowledge in case management, social work and counseling in a hospice setting
- Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations Skills and Abilities

- Salary: $43.38 - $53.02/hr
