Public Service Loan Forgiveness (PSLF) Eligible

This position qualifies for federal student loan forgiveness. After 10 years of qualifying payments while working full-time for a nonprofit employer, the remaining balance on your Direct Loans may be forgiven.

About the Role

Join Housecall Providers as a Hospice Social Worker - PRN in United States. Hospice Social Worker - PRN --------------------------------------------------------------- NOTE: This is a PRN role that will work 100% in the community and support the Portland Metro area. Candidates will need to have 5 weekdays of availability per month.

Key Responsibilities

Performs initial and ongoing psychosocial/bereavement evaluations of patients/family/caregiver and assists in the development/implementation of the plan of care.

Is a core member of the interdisciplinary group (IDG) and actively participates in IDG meetings for development and evaluation of the patient’s plan of care.

Implements a plan of care for each assigned patient/family/caregiver unit and evaluation of that plan of care.

Assists patient/family/caregiver in determining of goals of care including completion of Advance Directives and POLST forms as needed.

Requirements

Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle

CPR certification at hire or within 6 months in position Knowledge, Skills and Abilities Required Knowledge

Knowledge in case management, social work and counseling in a hospice setting

Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations Skills and Abilities

Salary: $43.38 - $53.02/hr

PSLF eligible (Public Service Loan Forgiveness).

Work Schedule

Per Diem position.

Flexible scheduling available.
