- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Active and unrestricted Arizona medical license or ability to obtain prior to start date
- Current and unrestricted DEA certificate or ability to obtain prior to start date The salary range for this role is $226,000 to $366,000 annually based on full
