Key Responsibilities
- Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment.
- Develop and implement tailored intervention plans based on assessment findings.
- Maintain accurate clinical records for all patients referred to social work services.
- Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs.
Requirements
