Key Responsibilities

• Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment.

• Develop and implement tailored intervention plans based on assessment findings.

• Maintain accurate clinical records for all patients referred to social work services.

• Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs.

Requirements

• on CPR certification. Valid driver's license and reliable, insured transportation. Excellent verbal and written communication skills, along with strong organizational abilities. #LI
