Identify patients at risk for medication non

adherence using centralized PHM reports, payer files, registries, refill history, and EHR tools.

Primarily support Medicare Advantage Star medication adherence measures while addressing adherence gaps across all populations.

Conduct patient outreach (phone, portal, text as appropriate) to address late refills, access barriers, and affordability concerns.
