Key Responsibilities

The Occupational Therapist will conduct in-home evaluations and deliver skilled therapy visits to build and execute patient care plans focused on functional independence. They are responsible for maintaining high-quality documentation, coordinating with the broader care team, and acting as a referral-source ambassador.

Requirements

Candidates must hold an active Michigan Occupational Therapist license and possess strong clinical judgment for independent field work. Reliable transportation, a valid driver's license, and experience in home health or related clinical settings are required.
