About the Role

Join Brownfield Rehabilitation and Care Center as a Licensed Social Worker (Part-Time) in United States. SOCIAL WORKER - PART TIME We are seeking a PT Licensed Social Worker who is able to dedicate around 8 hours per week to our center. The Licensed Social Worker plays a key role in supporting the emotional and social well-being of our residents.

Key Responsibilities

Assess and support the emotional and social needs of residents on an individual basis.

Maintain accurate documentation in resident medical records according to facility policies and federal/state regulations.

Collaborate with interdisciplinary teams to coordinate continuity of care.

Serve as a liaison between residents, families, and community agencies.

Requirements

Must be a Licensed Social Worker in good standing.

Completion of secondary education at an accredited school of Social Work.

Prior experience in a supervisory capacity is preferred.

Experience in geriatric, adult, or long

Work Schedule

Full-time position.
