About the Role
Join UW Health as a Clinical Pharmacist – Community Pharmacy – Float in United States. Work Schedule: 90% FTE, 36 hours per week. Day/Evening shift.
Key Responsibilities
- Provide medication therapy management services as part of the interdisciplinary team to patients to assure t.
- Coordinate information with physicians and clinic staff, documenting clinical information and outcomes within t.
- Provide training and education to pharmacy residents and PharmD students through practical and didactic expe.

Requirements
- Bachelor's Degree in Pharmacy or a PharmD degree Required
- Completion of an ASHP Accredited Residency training program Preferred Work Experience
- 1 year of clinical patient care and orders management experience Required
- 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred Licenses & Certifications
