- REQUIRED QUALIFICATIONS: Education: Doctorate Licenses/Certifications: Physician Licensed by the Federal Drug Enforcement Administration.

- Obtain and maintain medical staff membership and appropriate privileges.
- Must be eligible as a provider for Medicare, Medicaid, and other federal heath programs.
