Registered Nurse - Patient Navigator

Key Responsibilities

The Registered Nurse serves as a primary point of contact for patients and families to coordinate care and decrease service fragmentation. They also collaborate with medical teams to monitor patient progress, track quality metrics, and assist in the development of clinical pathways.

Requirements

Candidates must hold a Specialized Diploma in Nursing and possess at least 2 years of experience as a Registered Nurse. A valid Pennsylvania RN license is required upon hire, along with proficiency in Microsoft Office and knowledge of patient education techniques.
