About the Role

Join Goldbelt, Inc. as a Licensed Clinical Social Worker - Internal Medicine in United States.

Key Responsibilities

  • Provide direct and indirect services to both inpatients and outpatients in accordance with social work standards of practice.
  • Conduct psychosocial assessments to determine patient needs and resources (both family and community support).

Requirements

  • of Twelve 30 minute appointments per day, with a variety of routine scheduled and same day availability via warm hand-offs from providers.
  • Provide concise documentation of care and recommendations in patient’s medical record in no less than 72 hours after the patient is seen.
  • Not exceed approximately 16 sessions per day to include individual and group appointments.
  • Participate in group medical appointments including Shared Medical Appointments (SMA) and Drop in Group Medical Appointments (DIGMA) as requested by the PCM.
