About the Role

Join Goldbelt, Inc. as a Licensed Clinical Social Worker - Internal Medicine in United States.

Key Responsibilities

Provide direct and indirect services to both inpatients and outpatients in accordance with social work standards of practice.

Conduct psychosocial assessments to determine patient needs and resources (both family and community support).

Requirements

of Twelve 30 minute appointments per day, with a variety of routine scheduled and same day availability via warm hand-offs from providers.

Provide concise documentation of care and recommendations in patient’s medical record in no less than 72 hours after the patient is seen.

Not exceed approximately 16 sessions per day to include individual and group appointments.

Participate in group medical appointments including Shared Medical Appointments (SMA) and Drop in Group Medical Appointments (DIGMA) as requested by the PCM.
