About the Role
Join Parkland Health as a Hospital at Home Pharmacist in United States. Primary Purpose Provides pharmacy services to patients, nurses, physicians, and other medical staff by interpreting and evaluating the validity and safety of orders and prescriptions. Integrates core pharmacy knowledge dispensing and order review activities.
Key Responsibilities
- effective therapies and implements cost containment strategies such as therapeutic exchange programs.
- Educates and communicates effectively with patients on medication therapies to ensure that patients are informed of proper drug usage and knowledge of potential side effects.
- Documents interventions for routine resolved prescribing, formulary & therapeutic issues. Identifies & discusses adverse drug events with the care team and appropriately documents information via web
- based process.
Requirements
- Must have a bachelor's degree in pharmacy or Pharm. D degree from an accredited school of pharmacy. Experience

- None. Certification/Registration/Licensure
- Must be a Registered Pharmacist in the State of Texas. Skills or Special Abilities
