Key Responsibilities

Collaborate with the HART Hub multi

disciplinary health team

Complete comprehensive and ongoing assessment of client need.

Assess risk, develop strategies to mitigate risk, and implement safety plans where needed.

Requirements

Attend and participate in team/staff meetings, and all

staff meetings as required.

Participate in agency quality improvement activities, e. g. accreditation projects, committee work, special events.

Participate in quality improvement activities at the team/program level.

Benefits & Compensation

Salary: $32.38 per hour

Work Schedule

Full-time position.

Night or evening shifts may be required.
