Key Responsibilities

• Collaborate with the HART Hub multi

• disciplinary health team

• Complete comprehensive and ongoing assessment of client need.

• Assess risk, develop strategies to mitigate risk, and implement safety plans where needed.

Requirements

• Attend and participate in team/staff meetings, and all

• staff meetings as required.

• Participate in agency quality improvement activities, e. g. accreditation projects, committee work, special events.

• Participate in quality improvement activities at the team/program level.

Benefits & Compensation

• Salary: $32.38 per hour

Work Schedule

• Full-time position.

• Night or evening shifts may be required.
