Requirements

Required qualifications include a Pharm.D degree (or RPh with extensive experience), a valid CA pharmacist’s license, a minimum of 6 months of hospital pharmacy experience, and current American Heart Association BLS certification. Preferred qualifications include advanced life support certifications if working in critical care areas and completion of an ASHP residency program.
