The Behavioral Health Case Manager provides therapeutic services, develops individualized treatment plans, and coordinates care for clients. They also facilitate linkages to community resources, perform crisis prevention, and maintain positive relationships with external contacts.
Candidates must be at least 18 years old with a high school diploma and three years of relevant experience, or a Bachelor's/Master's degree in a related field. A valid driver's license, reliable transportation, and the willingness to travel within the service area are required.
