Key Responsibilities
- Ensures quality of care through formulary and/or medication management and quality improvement projects.
- Evaluates patient medical profiles for appropriateness of medication therapy based upon patient specific medical history, age, and physical status, and makes/documents drug therapy recommendations.
- Monitors, evaluates, and guides the provision of pharmaceutical care through staff/resident/student development and education.
- Supports patient care functions of the network, especially in area of specialty.
Requirements
- scheduled drugs and pharmacy security. BCPS
- Board Certified Pharmacotherapy Specialist
- State of Pennsylvania within 2 Years RP
- Registered Pharmacist
