Key Responsibilities
- Assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for occupational therapy.
- identifies patient needs for other disciplines and services.
- Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload.
- Communicates significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated.
Requirements
- Requirements: 2 years of experience as an occupational therapist Current registration to practice Occupational Therapy in New York State Salary Range USD $90,000.
- This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
