About the Role

Join WellSpan Health as a Case Management Social Worker - York Case Management - Day in United States. Schedule Full Time, Day Shift (80 hours biweekly) Hours: Monday - Friday, 8:00am - 4:30pm Rotation: Occasional weekends General Summary Plans, organizes and implements social work for patients and families. Works in a team model to address patient care needs.

Requirements

Candidates must possess a Bachelor's Degree in social work or social welfare as required by PA Act 68 of 2008, along with less than one year of relevant experience. Required licenses include Basic Life Support upon hire, with a Licensed Social Worker preferred upon hire.
