About the Role

Join CentraCare as a Senior Transitions Physician – $325K-$400K, Part-Time in United States. Join an expanding geriatric focused service line that provides mobile primary care services to multiple sub-acute, assisted living, and long-term care locations within a 60-mile radius of St. Cloud.

Key Responsibilities

will support our advanced practice providers in their onsite geriatric primary care practice for our long-term care patients by completing the physician required regulatory visits, rounding in our subacute settings, and help support our APPs with MD required services in our assisted living facilities.

Requirements

BE/BC in FM or IM

Eligibility for Minnesota medical licensure Our Location CentraCare, a leading not

bed regional referral center and Level II trauma center, delivers comprehensive inpatient and outpatient services with Magnet

designated nursing and expert support staff. Just 60 minutes from Minneapolis

Work Schedule

Part-Time position.

Call responsibilities required.

Weekend coverage may be required.
