The Primary Care Physician acts as the lead in a team-based care environment, focusing on providing an unmatched experience for seniors through a value-based model. This involves taking a hands-on approach to address patient needs inside the exam room and understanding social determinants of health affecting outcomes.
Candidates must hold a Doctor of Medicine or Osteopathic Medicine degree and be board certified or eligible in Geriatric, Family, or Internal Medicine. A valid, unrestricted state license and current, unrestricted DEA license are mandatory requirements for this onsite role.
