- Unrestricted Arizona Physician Assistant OR Nurse Practitioner license or ability to obtain prior to start date
- Physician Assistant national certification through NCCPA; OR National Nurse Practitioner certification through AANC or AANP
- Current DEA registration or ability to obtain prior to start date
