Key Responsibilities
- A day in the life of a Clinical Pharmacist I at Hackensack Meridian Health includes: 1.
- Identifies, resolves, and prevents drug-related problems.
- Recommends and monitors drug therapy.
- Participates in the department's drug formulary review process, including the preparation of drug monographs for review by the Pharmacy and Therapeutics Committee.
Requirements
- Education, Knowledge, Skills and Abilities Required: 1.
- Graduate of an ACPE accredited school of pharmacy with a minimum of a Pharm.
- Completion of a clinical pharmacy residency program or 5 years of hospital pharmacy experience with a minimum of three years in clinical practice.
