Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant or Nurse Practitioner with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being.
Key Responsibilities
- quality care to seniors in their homes, assisted living, hospice, and nursing facilities. Delivering dementia
- informed, relationship
- functional partners – including palliative, psych, and neuro – to deliver coordinated, whole
- person care. Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals. Maintaining consistency in documentation, follow
Requirements
- 2 years of clinical experience (geriatrics or primary care preferred). Compassionate, patient
