About the Role

Join ACCESS MEDICAL CLINIC as a Nurse Practitioner/PA – Long Term Care, Weekend Coverage in United States.

Key Responsibilities

  • manage acute and chronic conditions common in LTC populations.
  • Develop and adjust individualized care plans in collaboration with facility teams.
  • Manage high risk LTC needs including infections, wounds, CHF/COPD, dementia, and polypharmacy risks.

Requirements

  • The provider will be responsible for rounding in person on Saturday & Sunday to address acute issues and take call for the facilities during the weekend.
  • Typical Coverage hours are 5pm Friday until 8am Monday.
  • MSN, DNP, MPAS, DMSc, or equivalent advanced degree.
  • Active state licensure and board certification (AANP/ANCC/NCCPA).
