The provider develops and delivers personalized behavioral health care plans, including initial assessments and clinical interventions for older adults. They collaborate with an interdisciplinary team and external providers to manage behavioral complexities and prevent inpatient psychiatric hospitalizations.
Requires a Master's degree in behavioral health, social work, or psychology and a valid professional license (LCSW, LMFT, LPC, or Psychologist). Candidates must have at least one year of experience in an integrated care setting and current CPR certification.
