About the Role

Key Responsibilities

• Follow designated care plans based on patient needs.
• Make home visits within assigned geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor and report changes in client conditions.
• Execute and document doctors' orders.
• Perform client assessments and case management.
• Accurately document all care activities using electronic tablets.

Requirements

• Current Registered Nurse license in South Carolina.
• Graduation from an accredited nursing program.
• Home health experience required.
