• Assists with planning and implementing a comprehensive Social Services program.

• Works with the interdisciplinary team to promote and protect patients/residents' rights and the psychosocial well

• Complete a comprehensive Psychosocial Assessment for each patient/resident that identifies social, emotional, and psychological needs and strengths. Assesses each patient/resident for discharge.
