Key Responsibilities

  • Perform patient assessments, monitor therapeutic responses, and adjust medication regimens accordingly
  • Provide education and counseling to patients and caregivers on medication therapy, disease state management, and lifestyle modifications.
  • Conduct home visits as necessary to evaluate patient adherence, identify potential medication-related issues, and provide direct patient care.
  • Review and evaluate medication orders for appropriateness and effectiveness, making recommendations for therapy adjustments as needed
