About the Role

Join Edenbridge Health Llc as a Occupational Therapist – PACE Program, DC in United States.

Key Responsibilities

Delivery of appropriate occupational therapy evaluation and treatment to PACE participants on acute, restorative or maintenance levels as needed.

Performs timely initial and annual assessments for all participants and corresponding plans of care. Performs semi

annual reassessments and unscheduled change of condition assessments for all participants receiving Occupational therapy services as well as corresponding plans of care.

Participates in home safety evaluations as directed for potential enrollees into the program.

Requirements

Graduate of an Occupational Therapy program accredited by the American Occupational Therapy Association is required.

Must submit to and pass a pre

employment screening and criminal background check.

Must have a current Driver’s License and driving record must be in compliance with Company policy. Must maintain current automobile insurance.
