Key Responsibilities

Regimen review, ensuring safe medication administration, monitoring for desired therapeutic outcomes, identifying and avoiding medication errors and adverse drug events, etc.

Provides pharmacological information by answering questions and requests of health care professionals

Works collaboratively with Management staff in the medication management of patients transitioning from inpatient to outpatient or next level of care

Counseling patients on drug therapies

Requirements

Graduate of school of pharmacy with a Doctor of Pharmacy (Pharm. D.) degree.

Current licensure as a pharmacist in the State of California

Ability to exercise independent judgment and prioritize a diverse workload.

Outstanding organizational and communication skills.

Benefits & Compensation

Salary range: $125,000 to $130,000 annually.

Work Schedule
