Occupational Therapist- Home Health

Key Responsibilities

The Occupational Therapist will perform home visits to evaluate patient functional status and develop individualized intervention plans to improve independence. They will also collaborate with a multidisciplinary team to provide rehabilitative care and educate patients and families on health and safety.

Requirements

Candidates must hold a current VA Occupational Therapist license and have at least one year of recent work experience. Proficiency in documentation and the ability to work independently in a home care setting are required.
