- Reduce and mitigate psychosocial distress in patients and families.
- Improve continuity of psychosocial care across hospital settings.
- Link patients and families supported by the palliative care consultation service to outpatient clinics and resources.
- Provide post discharge contact to ensure discharge plan is secure and the continuation of additional palliative care support.
