Key Responsibilities

- will work closely with the clinical team and is primarily responsible for managing direct patient care with the implementation of evidence-based medicine and adherence to national and internal practice guidelines and policies.
- Will Focus On: Health promotion Disease prevention Health maintenance Counseling Patient education Annual Health Assessments (AHA) Chronic Care Management (CCM) Provides direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.

- Salary range: $250,000 to $325,000 annually.
