Key Responsibilities
- Provides patient
- Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
- Reviews charts, evaluates pertinent laboratory data, drug
- VA medications or discontinuing duplicate medications or those the patient is not taking after communicating with the provider
