Job Title: Telehealth Nurse Practitioner Job Location: Remote, *we only hire in certain states* MUST have a Texas NP License! Key Responsibilities
- will manage high-risk patients in a telemedicine capacity for patients in the Southern OR community with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients’ medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare.
- will you do as Nurse Practitioner at GGH? * Assess patients' healthcare needs and provide clinical care and consultation via telemedicine * Perform an annual wellness exam for senior Medicare patients via telemedicine.
- will have access to EMR and/or portals to review charts and document notes * Cover for other Nurse Practitioners and Care Team Members when necessary * Maintains patients’ confidence by completing and safeguarding medical records, keeping patients’ information confidential, and abiding by all HIPAA rules and regulations This position is a full-time Nurse Practitioner opportunity (5 days a week, 8am-5pm schedule).
