Key Responsibilities

• will provide emotional support, counseling, and resource coordination for patients and families during their hospice journey.

• Conduct psychosocial assessments, develop care plans, and document bereavement activities Design and distrib.

• Develop programs on grief and loss for families and staff Evaluate the bereavement program quarterly and ann.

Requirements

• days per week, 8:00 AM

• 5:00 PM Anticipated pay range $55—$65 USD Silverado offers the following
