RN Case Manager-Full Time

Key Responsibilities

The RN Case Manager plans, organizes, and provides hospice care while building community resources to meet the needs of patients and families. They also provide clinical direction to the interdisciplinary team and ensure compliance with patient care documentation and infection control standards.

Requirements

Candidates must hold a current Registered Nurse license and CPR certification, with at least two years of clinical, home health, or hospice experience preferred. A valid driver's license and reliable, insured transportation are required for community-based patient visits.
