Join Adaptive Home Health / Fira Health as a Physical Therapist – Home Health, Flexible Schedule in United States.

Key Responsibilities

will do: Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

Manage treatment and outcomes directly, including PTA supervision where appropriate.

Requirements

haves Prior home health experience Homecare Homebase (HCHB) experience (or similar home health EMR) Deeper clinical experience in high

signal PT populations Experience supervising/mentoring PTAs and collaborating on care plans Strong organization and time management

Salary: $100K - $130K

Work Schedule

Full-Time position.

Weekend coverage may be required.
