Key Responsibilities

A day in the life of a Unit Based Pharmacist at Hackensack Meridian Health includes: 1.

Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care.

Perform patient assessment and counseling: Develop individualized treatment and monitoring plans for patients.

Education, Knowledge, Skills and Abilities Required: 1.

Bachelor's Degree from an accredited college of Pharmacy.

Possess strong interpersonal skills and work well at the team level, as well as independently.

Excellent written and verbal communication skills.
