Key Responsibilities
- A day in the life of a Unit Based Pharmacist at Hackensack Meridian Health includes: 1.
- Validate medication reconciliation: Reconcile medications upon admission for admitted patients.
- Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care.
- Perform patient assessment and counseling: Develop individualized treatment and monitoring plans for patients.
Requirements
- Education, Knowledge, Skills and Abilities Required: 1.
- Bachelor's Degree from an accredited college of Pharmacy.
- Possess strong interpersonal skills and work well at the team level, as well as independently.
- Excellent written and verbal communication skills.
