About the Role
Join Jennie Stuart Health as a Behavioral Health Social Worker – Full Time in United States. Jennie Stuart Health is seeking a BHU Social Worker to join our Behavioral Health Unit. This role is responsible for supporting patients through psychosocial assessment, treatment planning, therapy participation, and discharge coordination.
Key Responsibilities
- Conduct psychosocial interviews and gather social history information relevant to treatment and discharge planning
- Assess social factors affecting the patient’s condition and modify service plans as needed
- Participate in Treatment Team meetings and contribute to patient care planning
- Meet with patients and families in individual or group settings as requested
Requirements
- Collect and compile data, complete reports, and support quality improvement initiatives
- Collaborate with hospital staff and outside resources to support continuity of care Qualifications:
- Master’s degree in Social Work from an accredited Graduate School of Social Work required
- Social Worker licensure required
