About the Role

Join St. Cloud Hospital as a Social Worker – Float Pool, M-F Schedule in United States. Join the inpatient Care Management team at St. Cloud Hospital in the role of Float Social Worker.

Key Responsibilities

  • will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.
  • provide supervision for new grads in-house or will reimburse up to $175/month for external supervision.

Requirements

  • Current Licensed Social Worker (LSW) in Minnesota required.
  • Master's degree in Social Work or Sociology required.
  • At least two years of experience as a Social Worker, preferably in a healthcare setting.
  • American Case Management Association (ACMA) Case Manager-SW Certification required within five years.

Benefits & Compensation

  • Salary: $63.8K - $103.4K
  • Signing bonus: $3,000.
  • Relocation assistance: $5,000.

Work Schedule

  • Full-Time position.
  • Day shift only - no nights.
