About the Role
Join PacificSource as a PACE Physical Therapist – $75K-$123.7K in United States. Looking for a meaningful way to make a difference in the lives of older adults? At Pacific PACE LLC, you will be part of a team that provides compassionate, coordinated care that helps participants live safely, independently, and with dignity.
Key Responsibilities
- Provide Physical Therapy treatment and interventions to PACE participants across the continuum, including in the home and/or community setting.
- Conduct comprehensive assessments of participants to determine their functional abilities and needs.
- Actively participate in the interdisciplinary team by offering clinical expertise in team planning, problem-solving, and processes.
- Develop and implement personalized care plans aimed at improving or maintaining participants’ independence and quality of life.
Requirements
-, including attendance at team and staff meetings, and recording and tracking PT rehabilitation equipment.
- Visit participants in their homes or care settings as needed.
- Serve on various internal and external committees as required or designated.
- Provide all services in a manner consistent with State and Federal regulatory requirements.
