Key Responsibilities

provide community-based rehabilitation services to participants enrolled in the ABI/MFP Waiver programs.

develop and implement individualized therapy plans focused on maximizing independence, safety, and functiona.

Conduct functional assessments and evaluate participants’ strengths, goals, and rehabilitation needs Develop.

Provide therapeutic interventions addressing: Personal management skills Communication and cognitive functio.
