Key Responsibilities

Conducting and evaluating medication histories, assessing compliance and suggesting modifications so as to achieve desired therapy outcomes.

This role will provide educational assistance to patients/families/caregivers to ensure understanding and to provide information regarding the patient’s therapy/treatment.

Management oversight of the therapy care plan, including risk-based care assessments and follow up clinical interventions as specified in client agreements.

Benefits & Compensation

Salary range: $125,000 to $135,000 annually.

Requirements

Candidates must possess a degree in pharmacy from an accredited school, with a Pharm. D. degree preferred, and must be licensed in the state of Illinois, with multi-state licensure preferred. A minimum of two years' experience as a licensed pharmacist and intermediate computer skills are required.
