- Preforming psychosocial and mood assessments.
- Following up on needs determined in assessment and care planning for dc, mood, behaviors, and psychosocial issues.
- Preforming ongoing care, discharge, and transition planning and coordination.
- Arranging for services, referrals, and providing resources to patients. Referring for home and outpatient follow up care, and placement and transfers for assisted living.
Requirements
- Bachelor’s degree in Social Worker
- LSW or higher
